When to Use This Checklist
If you're the person who signs off on medical equipment orders — cardiac monitors, sleep diagnostic devices, CPAP machines — this checklist is for you. I've managed purchasing for a multi-specialty clinic since 2020. That's roughly $340,000 a year across about a dozen vendors. I've ordered the wrong device, paid for expedited shipping I didn't need, and signed for a delivery that was missing half its accessories.
I won't pretend to be a clinician. I'm an administrator who got tired of explaining to finance why unused equipment was sitting in storage. This is the 7-step checklist I now run every major purchase through. It's not exhaustive, and it's not a substitute for good clinical advice. But it will catch the problems that are entirely preventable from the buying side.
Step 1: Define the Clinical Use Case Before You Look at Specs
It's tempting to start with product brochures. Resist. The specs only matter in the context of who's going to use the device, where it's going to live, and which patients it will serve.
I went back and forth between a compact spot-check cardiac monitor and a full telemetry system for about two weeks. The spot-check unit offered flexibility and a much lower price. The telemetry rack offered continuous monitoring. We ultimately chose the portable unit because our patient volume didn't justify the installation and training costs of a full telemetry setup. If you're a 400-bed hospital, that calculus is entirely different.
Write down the answers to these before contacting any vendor:
- Which department gets the device, and how many staff will operate it?
- What measurements or functions are non-negotiable?
- Is this a replacement, or a brand-new capability for the facility?
We skipped the 'new capability' question once, and it cost us. The device was fine — but we had no workflow for maintaining it, and it sat unused for four months.
Step 2: Verify Regulatory Status Early
For any cardiac monitor or sleep diagnostic device, check regulatory status before you get deep into pricing. In the U.S., that means FDA clearance or approval. In Europe, it's CE marking. This isn't paperwork bureaucracy — it determines whether you can legally use the device in a clinical setting.
Ask the vendor for the clearance letter or certificate number. A reputable manufacturer sends it without hesitation. Vague answers? That's a red flag.
Also check that accessory items — cables, sensors, cuffs — are covered under the same clearance. I've seen devices that were cleared but had replacement parts that weren't legally marketed for use with them. Not ideal.
Step 3: Understand How the Device Actually Works
You don't need to be a biomedical engineer, but you need enough understanding to verify what you're ordering. This is where first-time buyers get tripped up, especially with respiratory and sleep equipment.
Take CPAP machines. How does a CPAP machine work? It draws room air, filters it, and delivers it through a hose and mask at a continuous pressure. That pressure acts as a pneumatic splint, holding the upper airway open while the patient sleeps. It doesn't breathe for the patient — it just keeps the airway from collapsing. The pressure stays constant, which is the 'continuous' part of continuous positive airway pressure.
Many people assume CPAP and BiPAP are interchangeable. They're not. BiPAP offers two pressure settings — lower on exhalation, higher on inhalation — and it's often prescribed for patients who can't tolerate a single pressure level or who have certain respiratory conditions. Ordering the wrong one means a return, a restocking fee, and a clinician waiting an extra two weeks for the right device. Know the difference before you sign.
Same logic applies to sleep diagnostic devices. A home sleep test device that records oxygen saturation, heart rate, and airflow is not the same as an in-lab polysomnography system. They have different clinical uses, different regulatory pathways, and very different price tags. Both are legitimate 'sleep diagnostic devices' — but if your staff expects one and you buy the other, the purchase is scrap.
Step 4: Check Compatibility with Your Existing Infrastructure
This is the step most people skip. I've seen a cardiac monitor with perfect specs get returned because it couldn't connect to the facility's EMR. Not 'didn't connect well' — couldn't connect at all. The vendor never asked, and the buyer never thought to check.
Before you order, confirm:
- Does the device integrate with your EMR, or does data have to be documented manually?
- What connectivity does it use (HL7, Wi-Fi, Bluetooth)?
- Who handles installation — your facilities team or the vendor?
- Are the consumables standard stock items, or a new supply chain you're creating?
The last one has stung us more than once. We bought a sleep diagnostic device that uses proprietary sensors, which meant a separate purchasing contract for the rest of its life. Workable, but I would have preferred to know that before the commitment.
Step 5: Evaluate the Service Footprint, Not Just the Brand Name
A device is only as good as the technician who shows up when it fails. Before finalizing any order, search for the manufacturer's service coverage in your area.
For example, if you're in the Southeast and evaluating Hamilton Medical equipment, you might find Hamilton Medical Ringgold listed as the nearest service location. In the New Jersey area, similar searches might surface AZZ Medical Hamilton NJ as a local option. The point isn't which name shows up — it's that you verify a qualified technician can actually reach your facility within the response time your contract requires.
Ask three questions: What's the guaranteed response time? Is loaner equipment available during extended repairs? Are preventive maintenance visits included in the contract, or billed separately?
I can only speak to our region and our facilities. If you're in a different part of the country, there are likely local options I'm not aware of. The key is confirming coverage before you need it — not after a monitor goes dark.
Step 6: Calculate Total Cost of Ownership, Not Sticker Price
The purchase price is roughly half the story. When we reviewed our equipment spending last year, we found we'd spent about $32,000 on a cardiac monitor package — maybe $29,000, I'd have to check the exact PO — and then another $6,000 in the first 18 months on service, calibration, and replacement accessories.
Compare the full package, not just the devices:
- Warranty length, and what it excludes (consumables are usually excluded)
- Preventive maintenance visit costs and intervals
- Calibration schedule and fees
- Accessory lifespan and replacement pricing
A slightly higher quote can be the cheaper option if it includes a longer warranty and lower periodic service costs. Don't get fooled by the line item that sits at the top of the quote.
Step 7: Use a Delivery-Day Checklist
This step has saved us more times than I can count. When equipment arrives, have the checklist printed and run through it with the delivery or installation team before you sign anything. We learned this after a vendor delivered a sleep diagnostic device with the wrong accessories — the packing slip said all parts were there, but it wasn't until the technician went to use the device that we found the sensors didn't match.
Delivery-day essentials:
- Serial number on the device matches the packing slip and your PO
- Every standard accessory is present and is the correct model
- Software license keys, logins, or physical keys are included
- Device powers on and completes its startup self-test
- Packaging is intact, with no signs of shipping damage
Five minutes of verification beats five days of correction.
Common Mistakes to Avoid
Here are the four mistakes I've made myself or helped clean up:
Treating regulatory status as a back-office detail
If the device isn't cleared for your market, it can't legally be used in patient care. No contract language can fix that.
Confusing CPAP with BiPAP
They don't serve the same patients. The clinical team will specify which one is appropriate. Your job is to verify the order matches the request.
Assuming installation and training are included
Some vendors deliver a box and leave. Wall mounting, network setup, and staff training are often separate line items. Get them itemized in writing.
Waiting until the budget deadline
I once had two hours to decide before our fiscal year-end cutoff. Normally I'd compare three vendors and get a clinical reviewer to check the specs. There was no time. I went with our usual supplier and took the spec sheet at face value. It worked out, but that was luck, not process.
A Note on Scope
This checklist reflects what has worked for us at a mid-sized outpatient clinic with predictable ordering patterns. If you're running a larger system with a dedicated clinical engineering team, some of these steps will feel basic. If you're at a small clinic, some will feel like overkill. Use what fits, and ignore the rest.
The idea that ties the whole list together is simple: checking first is cheaper than reordering. It's the one procurement cost that's always worth paying.
