Common Mistakes to Avoid
I review roughly 200 medical device deliveries a year. And I'll be honest—about 15% of first deliveries had something wrong in Q1 2024. Not always obvious stuff. Sometimes a spec number off by 0.5mm. Sometimes a sterilization indicator that wasn't clearly marked.
Here's the thing: 5 minutes of verification at the loading dock can save 5 days of correction later. That's not a theory—that's a $22,000 redo I managed in 2023 because someone accepted a batch of ventilator components without checking the packaging spec against our contract.
So here's the checklist I use. It's not complicated. But it works.
When This Checklist Applies
Use this checklist when you're accepting delivery of: critical care ventilators, ICU monitors, molecular diagnostic platforms, infusion pumps, sterilization equipment, surgical instruments, or any regulated medical device component.
If you're accepting something like general office supplies, you can relax a bit. But for anything patient-facing or lab-critical? Use the checklist.
There are 8 steps. Do them in order.
Step 1: Verify the Packing Slip Against Your PO
First thing: pull the packing slip. Match it against your purchase order line by line.
I'm looking for:
- Product codes matching exactly (not 'equivalent' or 'upgraded'—exact match).
- Quantities correct. Sounds basic, but I've seen a lot of partial shipments called 'fulfilled' on paper.
- Lot numbers if applicable. For sterile items, lot numbers need to match the certification documents.
Quick check: Do the product codes on the packing slip match your PO exactly? If any character is different, flag it. Don't assume it's a typo.
Step 2: Inspect Packaging Integrity
Before you open anything, inspect the outer packaging.
Things I look for in my Q1 2024 audit that caught problems:
- Crushed corners or compression damage. This can compromise sterile seals.
- Water damage or staining. Even a small amount of moisture in transit is a red flag.
- Tamper-evident seals—are they intact? If not, reject the shipment immediately.
- Label legibility. Sounds small, but if labels are smudged or torn, traceability is compromised.
In 2022, I rejected a batch of 80 infusion pump consumables purely because the outer packaging had visible water damage. The vendor pushed back. But when we opened a sample, the internal sterile pouches had condensation. That was the right call.
Step 3: Check Product Condition and Sterility Indicators
Now you open the inner packaging. For sterile medical devices, this step is non-negotiable.
Verify:
- Sterilization indicators (chemical indicator strips, autoclave tape, or indicator dots) have changed color properly. If they haven't, do not proceed.
- Expiration dates on sterile packaging. I've received shipments where 60% of units had less than 6 months shelf life—fine for some use cases, but not if these are for a stock rotation.
- No visible contamination, discoloration, or damage to the product itself.
Experience note: I've only worked with sterile device shipments from major manufacturers (Hamilton Medical being one). If you're dealing with smaller suppliers or re-sterilized devices, your inspection needs to be even more thorough.
Step 4: Verify Specifications Against Your Contract
This is the step most people skip, and it's the one that costs the most when you get it wrong.
Pull out your spec sheet or contract. Check:
- Dimensions, if applicable. I've seen ventilator tubing labeled '1.8m' come in at 1.75m. A 5cm difference matters in ICU configurations.
- Material specifications. Is the plastic medical-grade PVC, or standard PVC? There's a difference, and the price difference is substantial.
- Connector types. For infusion pumps, a Luer lock vs. a catheter tip is a completely different product.
When I compared our Q1 and Q2 results side by side—same vendor, different specifications—I finally understood why checking dimensions at delivery matters so much. We had a $3,000 discrepancy in a single shipment just from a connector spec that wasn't caught.
Step 5: Check Documentation Completeness
Medical device deliveries come with documentation. Without it, you can't legally use the product in most jurisdictions.
You need:
- Certificate of Conformance or Compliance. Must match the lot number of the shipment.
- Sterilization certificate (for sterile devices). Dated and signed.
- Declarations of Conformity if CE-marked or FDA-registered.
- User manuals or IFU (Instructions for Use) in the required language, if applicable.
Hard lesson: In early 2023, we accepted a shipment of diagnostic reagents—about $18,000 worth—without verifying the Sterilization Certificate. Turned out the sterilizer cycle used didn't meet our specified F0 value. We had to quarantine the entire batch, re-test, and ultimately absorb the cost. All because a piece of paper wasn't checked at the door.
Step 6: Sample Test Functional Units
You can't test every unit—that's impractical. But you should have a sampling plan.
I typically sample 5% of a batch for functional testing. For smaller batches (under 50 units), I test at least 3.
What to check:
- Does it power on and operate according to specs?
- Are all alarms and indicators functional?
- Do settings respond correctly?
- Are there any unusual noises or vibrations?
If even one sample fails, I don't accept the batch. I quarantine the entire shipment and request the vendor's corrective action plan. That happened last year with a monitor batch—2 of 50 had screen flickering. The vendor thought it was acceptable 'within specifications.' I didn't.
Step 7: Record and Photograph Any Discrepancies
If you find something wrong, document it immediately.
Take photos of:
- Damaged packaging
- Labels with errors
- Spec differences (show a ruler or comparator if applicable)
Write a discrepancy report with: shipment reference, date, time, what's wrong, and the spec it violates. Send it to your vendor and your procurement team the same day.
The 12-point checklist I created after my third mistake has saved us an estimated $8,000 in potential rework just from documentation alone. Vendors respond differently when you have a dated photo and a contract clause to back it up.
Step 8: Confirm Acceptance or Quarantine
Once you've completed steps 1-7, you make a decision:
- Accept: All checks passed. Release the shipment to inventory or clinical use.
- Quarantine: Any check failed. Tag the entire shipment as 'QUARANTINE' with the date and reason. Do not use. Do not distribute.
If quarantined, notify your quality team and the vendor within 24 hours. Set a timeline for resolution—don't let quarantine become indefinite storage.
Common Mistakes to Avoid
1. Trusting the vendor's 'we checked it already.'
I've heard this 100 times. And I've rejected shipments that the vendor swore were 'perfect.' Your check is your responsibility. Do it.
2. Spot-checking without a plan.
Random spot checks aren't a sampling plan. Use a structured approach (like the 5% minimum I mentioned) to make sure your sample is representative.
3. Accepting a shipment 'under protest' to keep things moving.
This is the most expensive shortcut. Accepting a bad shipment to meet a deadline means you're now managing non-conforming inventory, not using good inventory. It takes longer and costs more than rejecting at the door.
4. Skipping Step 4 (spec verification) because 'it's the same as last time.'
It might be. But I've caught spec changes that weren't communicated—a change in connector type, a different material grade—because I checked. Vendors change things sometimes without telling you.
5. Not photographing discrepancies.
A photo is worth a thousand words in a vendor dispute. I learned that when I couldn't prove a damage claim because I hadn't taken pictures. Now I photograph every issue, every time.
That's the checklist. It's not flashy, but it's what I use.
